تقييم أداء المستشفيات

Abstract

جاءت هذه الدراسة من أجل تقييم مستوى الأداء في المؤسسات العمومية الاستشفائيةبالجزائر (EPH)، من خلال مؤشرات تقييم آداء النظم والمؤسسات الصحية، في ظلالتغيرات الهيكلية التي مست منظومة الصحة لضمان التغطيةالصحية الشاملة والتحسين المستمر لجودة خدمات الرعاية الصحية. وتحقيقا لأهداف الدراسة، تم الاعتماد على المنهج التحليلي للتعريف ببيئة المستشفيات، نظرا لأهميتها في تقديم خدمات صحية شاملة ومتنوعة، مرورا بمفاهيم إدارة الصحة والمستشفيات وأهم المبادئ والركائز التي يقوم عليها القطاع الصحي. كما تم تقييم الأداء الصحي بالمستشفيات وتوضيح أهم المتغيرات ذات الأثرعلى العملية، إضافة لأهم النماذج والمؤشرات المتبعة في تقييم الأداء. أما في الجانب التطبيقي، تم الاعتماد على المنهج التجريبي في حساب مؤشرات تقييم الأداء، من خلال تجميع وتبويب معطيات مكتب التنظيم ومتابعة النشاطات الصحية التابعة للمؤسسة العمومية الاستشفائية سيدي عيسى -ولاية المسيلة-خلال الفترة الممتدة من 2016م إلى 2020م. توصلت الدراسة إلى أن المؤسسة العمومية الاستشفائية محل الدراسة تعاني من اختلالات في استغلال الموارد المادية والبشرية، حيث حققت معدلات منخفضة الكفاءة في كثافة توزيع الموارد البشرية على الموارد المادية (الأسره)، بالإضافة لقصور عدد الأسِرَه في مصلحة طب الأطفال مقارنة بالإقبال الكبير للمرضى، انخفاض الإقبال على الخدمات الصحية لقسم الطب الداخلي وارتفاع معدل دوران مصلحة الأمومة وطب النساء والتوليد. أما من خلال نتائج الأداء الصحي عموما، يمكن القول أن المؤسسة تحقق نتائج مرضية على مستوى التحسين الصحي، وذلك بالحد من مسببات العجز والوفيات. This study comes in order to assess the level of performance in public hospitalinstitutions in Algeria (EPH), through the indicators of performance in health systems and institutions, in light of the structural changes that affected the health system to ensure comprehensive health coverage and continuous improvement of the quality of health care services. To achieve the objectives of the study, the analytical approach is used to define the hospital environment, given its importance in providing comprehensive and diverse health services, passing through the concepts of health management and hospitals and the most important principles and pillars upon which the health sector is based. The health performance in hospitals is also evaluated along with the most important variables that have an impact on the process are clarified, passing through the most important models and indicators used in evaluating performance. As for the applied work, the empirical method is relied on to calculate the performance evaluation indicators, by compiling and tabulating the data of the Regulation Office and following up on the health activities of the Public Hospital Institution of SidiAissa - M'sila - during the period from 2016 to 2020. The study concludes that the public hospital institution under study suffers from imbalances in the exploitation of material and human resources, as it achieved low rates of efficiency in the intensity of distribution of human resources to material resources (the family), in addition to the shortage in beds in the Paediatric Department compared to the high turnout of patients, low turnout for Health services of the internal medicine department and high turnover of the maternity and obstetrics and genecology services. As for the results of health performance in general, this institution achieves satisfactory results in terms of health improvement, by reducing the causes of disability and death.

Cette étude vise à évaluer le niveau de performance des établissements hospitaliers publics en Algérie (EPH), à travers les indicateurs de performance des systèmes et institutions de santé, et avec leschangements structurels qui ont touché le système de santé pour assurer une meilleure couverture des soins de santé et l'amélioration continue de la qualité des services de santé. Pour atteindre les objectifs de l'étude, l'approche analytique est utilisée pour définir l'environnement hospitalier, vu son importance dans la prestation de services complètes et diversifiées, en passant par concepts de gestion de la santé et des hôpitaux et les principes et les piliers les plus importants sur lesquels il est basé le secteur de la santé.Nous avons aussi évalué Le rendement en matière de santé dans les hôpitaux, et clarifier les variables les plus importantes qui ont un impact sur le processus, aussi les modèles et indicateurs les plus importants utilisés pour évaluer le rendement. En Ce qui concerne le cas pratique, nous avons calculé les indicateurs d'évaluation de la performance, en collectant et en compilant les données du Bureau de régulation et en faisant le suivi des activités de santé de l'hôpital Public de Sidi Aissa - M'sila - pendant la période de 2016 à 2020. L'étude conclut que l'établissement hospitalier public étudié souffre de déséquilibres dans l'exploitation des ressources matérielles et humaines, car il a atteint de faibles taux d'efficacité dans l'intensité de la distribution des ressources humaines aux ressources matérielles (la famille). en plus de la pénurie de lits dans le département pédiatrique par rapport à la forte participation des patients, faible participation pour les services de santé du département de médecine interne et un taux de roulement élevé des services de maternité et d'obstétrique et de généalogie. Quant aux résultats de la performance en matière de santé en général, cette institution obtient des résultats satisfaisants en matière d'amélioration de la santé, en réduisant les causes d'invalidité et de décès.
